Hello! I am writing to report an urgent and serious safety failure involving my 2025 Acura MDX A-Spec and to request immediate investigation and action. The vehicle’s driver-assist systems - specifically the lane departure warning and forward collision warning - failed to operate. In addition, both the steering wheel and the accelerator became stuck, causing my vehicle to collide head‑on with oncoming traffic. These failures placed my life and the lives of others in immediate danger. I previously opened a case with Acura Contact and Client Relations (Case #16550271). To investigate my vehicle failures that caused this major accident, the response I received was inadequate and deeply concerning. During my conversation with Acura representatives, I was told verbally without any willingness to provide this statement in writing that Acura “has nothing to do with inspecting their own car.” Instead, I was instructed to have my vehicle inspected by my insurance company, despite the fact that they are not certified to evaluate Acura safety systems and do not possess the diagnostic equipment required to assess failures of this nature. Given the severity of the malfunction and the resulting collision, I am requesting that Acura immediately arrange a proper inspection by qualified Acura technicians and provide a written explanation of how such critical safety and control systems could fail simultaneously.

The car sometimes brakes on its own, and sometimes it brakes more or less than normal. Also, sometimes when I try to turn it on, it struggles to do so.

My new Acura MDX has very soft braking under normal braking and with harder braking the pedal normally is at the same level as the accelerator pedal which I feel could be a safety issue if your foot is then pressing both pedals at the same time. Also, when driving with adaptive cruise activated, when the car starts to brake on its own following another slower car, with my foot resting on the accelerator, the brake petal will activate and sometimes trap my foot between brake and accelerator petals. This is new to Acura as this is my third MDX since 2014. It has been noted on u-tube videos of the new MDX's. The dealer says it is not an adjustable brake system. The car can be hard to stop with a heavy load. Nothing I have ever experience before with any car. On June 3, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE22005 after the agency received VOQs and several field reports concerning the Auto Idle Stop (AIS) feature on 2016-2019MY Honda Pilot vehicles. The complaints allege that the engine fails to restart on its own from a complete stop at a traffic light or road intersection with the AIS function engaged. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (“PGM-FI”) software update. The second stage is only deployed if the software update fails to remedy the issue. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. This Engineering Analysis is being opened to further consider the potential safety defect, including gathering additional data regarding Honda’s service campaign efficacy. ODI is additionally expanding the scope to encompass all vehicles addressed in Honda’s service bulletins 23-008 and 23-009 as well as newer model year vehicles, which are substantially similar to the vehicles covered by the service campaign. To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.
